Reporting to the Rehab Program Manager plans & administers occupational therapy treatment for patients suffering from injuries or diseases to muscle nerves joints or bones to restore function relieve pain & prevent disability. Requirements: Bachelor's degree in field of clinical practice Occupational Therapist - NH CPR Certified or within 30 days of hire Drivers License - Valid Major Responsibilities: Ensures patients have timely and accurate orders prior to delivering services. Review Physician's referral (prescription); patient's condition and medical record. Administer; score and interpret specialized testing techniques based on the diagnosis to determine treatment required. Completes a comprehensive assessment; including Oasis if applicable; per regulations. Develops and implements individual care plans with measurable functional goals; based on patient interests and abilities; in conjunction with physician and interdisciplinary team. Continually evaluate and modify goals as necessary and update physician and care team members. Provides practice in functional; prevocational; vocational and homemaking skills and activities of daily living. Coordinates participation in sensorimotor; educational; recreational and social activities designed to help patients or disabled persons develop or regain physical or mental functioning or adjust to disabilities. Select constructive activities suited to individual's physical capacity; intelligence level and interest to upgrade individual to maximum independence; prepare individual for return to community; assist in restoration of functions and aid in adjustment to disability. Designs and constructs special equipment for patient and suggests adaptation of patient and of patient's living environment. Orients; instructs and directs work activities of assistants; aides and students as appropriate. Provides training to staff and community as requested. Completes documentation on same day as visit; per agency standards. Meets agency expectations for weekend; holiday and on-call coverage and expected productivity standards.
